    Socially Determined, Acentra Health Team to Transform State Health Analytics

    Socially Determined and Acentra Health have partnered to help state healthcare agencies better predict risks, target interventions and lower program costs.

    The partnership allows Acentra Health to use Socially Determined’s nationwide social risk data and risk scores to extract deeper insights from Medicaid claims. This strengthens Acentra Health’s artificial intelligence and analytics infrastructure, enabling real-time intelligence and analysis to support population health.

    “Our partnership with Socially Determined adds critical context to our clinical insights, strengthening our AI and predictive analytics models,” said Sean Harrison, chief AI and analytics officer at Acentra Health. “By enriching Medicaid data with social risk factors, we’re helping states move from reactive care to proactive, targeted interventions that improve outcomes and reduce costs across populations.”

    Acentra Health’s “whole person, whole population” model combines community risk factors, individual social risk data and state-based claims analytics into a single health informatics platform. It delivers deeper insights into the complex factors driving health outcomes, going beyond traditional claims analysis.

    Powered by advanced analytics and AI, the platform gives healthcare providers actionable intelligence to guide next steps for at-risk groups and improve outcomes across diverse populations.

    Through its partnership with Socially Determined, Acentra Health will integrate nationwide social risk data into its analytics solutions to:

    • Identify high-risk cohorts: Helping states target interventions for individuals and groups most at risk.

    • Expand whole-person integrated care: Analyzing cost, quality and outcomes using a 360-degree view of person and community data.

    • Strengthen decision-making: Equipping public sector agencies with faster, data-driven strategies to improve member health.
