At the 2012 Annual HIMSS Conference and Exhibition, held in Las Vegas from February 20th to 24th, Cognosante, a leading healthcare IT provider, showcased its new transformation solution called eSante SmartStream.

“We are truly connecting the dots with eSante™ SmartStream™, bringing the right information to the right place at the exact time it is needed,” said Cognosante CEO, Michele Kang. “eSante SmartStream tracks and monitors patient care in real time for Accountable Care Organizations and public health initiatives. We are excited to demonstrate how this product will assist in revolutionizing the health information exchange arena.”

eSante SmartStream (patent-pending) collects, integrates and then filters clinical and administrative data to patients in real-time.   Alerts, reminders and other messages can also be triggered when there is need for swift clinical intervention.
